Remove the iCloud account. Go to Settings>Apple ID>Sign Out
If the device does not turn on, the lock can be removed by following these instructions.
Restore to factory settings. Go to Settings>General>Transfer or Reset iPhone>Erase All Content and Settings.
Include the note with your name and the PIN number for the device unless it is restored to factory settings.
Package the device carefully so it does not get damaged in transport.
